Bracing plays a vital role in the non-operative treatment of adolescent idiopathic scoliosis (AIS), especially when detected early. Recent high-quality studies confirm that bracing can alter natural progression curve, potentially preventing need for surgery. The concept is based on ability external force to guide spine growth, which has been experimentally proven. However, cannot straighten spine; its primary goal halt curve’s progression. evidence strong, with showing approximately two-thirds AIS curves be controlled through bracing. there are concerns about broad application indications, leading unnecessary some patients. Identifying 25% patients who will benefit from remains challenge. most effective applied between 25-45° during rapid growth phase, but compliance crucial success. choice brace type matters less than quality and corrective effect. practice requires close orthotist-surgeon relationship, follow-up visits essential monitor progress.
